Recognition Program: A Let’s Go! Practice
A Let’s Go! Practice must meet the three basic components of the healthcare program:
1 – Connect to your community and the Let’s Go! community efforts by hanging a Let’s Go! poster in your waiting and exam rooms.
2 – Accurately weigh and measure patients by determining Body Mass Index (BMI), BMI percentile, and weight classification in patients more than two years of age. Use these charts: Girls Boys
3 – Facilitate respectful conversations with your patients around weight by using the 5210 Healthy Habits Questionnaires: (2-9 yo) (10-18)
As a Let’s Go! Practice you will receive a framed certificate, be listed on the Let’s Go! website, and a letter will be sent to your practice’s senior leadership. We recognize practices annually in the fall.
